STEP 1
Request an information package by contacting the HIGH FIVE^® Ontario Office at 416.426.7286 or by email at HIGHFIVE@prontario.org. State your name, contact information, and why you are interested in HIGH FIVE^®.

STEP 2
You will be required to read your Organization's applicable HIGH FIVE^® Agreement and sign the related forms. Upon completion please submit them to the HIGH FIVE^®Ontario office by fax at 416.426.7371 or by mail at the address seen below.

STEP 3
Upon receiving an invoice from HIGH FIVE^® Ontario, submit your Annual Fee. Please make the cheque payable to:
Parks and Recreation Ontario
1185 Eglinton Avenue East, Suite 406
Toronto ON, M3C 3C6

STEP 4
HIGH FIVE^® Ontario will confirm your Organization's status.
